Will mesh actually fix slow WiFi upstairs?

Properly placed and wired mesh, yes. Plug-in repeaters usually make little real difference.

Do cables always need running for this to work?

A wired backhaul is best where it's possible. Where it isn't, we use good-quality wireless backhaul and position units carefully instead.

Can a garden office be covered as well?

Yes - either buried Ethernet or a dedicated point-to-point link across the garden.
